Geriatric Care Management Profession:

An Aging Life Care Professional, also known as a Geriatric Care Manager, is a health and human services specialist who helps families, guardians, physicians, attorneys and trusts who are caring for older relatives or clients with disabilities. The Aging Life Care Professional is trained and experienced in one or more fields related to long-term care, including nursing, gerontology, social work, or psychology, with a specialized focus on issues related to aging, elder care, and living with disability.
Critical Care Management provides an experienced, Florida-licensed Registered Nurse certified in Care Management to oversee the needs of those of all ages living with disease, disability or advanced aging. For individuals needing assistance with chronic medical, social and functional concerns, Critical Care Management is able to help set realistic goals and find solutions to meet those goals. The professional Care Manager is dedicated to enabling the client to remain as independent as possible, living life on their own terms; and is experienced at finding resources for clients and their families. As a Registered Nurse, the RN Care Manager is trained and experienced at coordinating between the client and their entire support systems such as family, physicians, caregivers, pharmacy, accountants, attorneys, and any significant others involved in the client’s social network.
Care management enables clients to live in the environment of their choice, with appropriate care-givers and under the supervision of an experienced team leader. A Registered Nurse Care Manager has the advanced training and background necessary for orchestrating the medical management of clients with complex clinical conditions.
